Job summary

Confluence Health is seeking a pharmacist to deliver high-quality outpatient pharmaceutical care across our service areas. You will perform patient consultations, conduct clinical reviews, and perform Drug Utilization Reviews for dispensed medications.

The role includes Tele-Pharmacy with remote verification and on-site backup coverage, plus therapeutic assessments for individuals and populations. You will report to the Pharmacy Manager and contribute to quality initiatives and protocol

Qualifications

• Pharmacist must be licensed to practice in Washington state.
• BLS certification required within first year of employment and recertified bi-annually.
• Basic computer skills including Windows, email, and password management.

Responsibilities

• Deliver outpatient pharmaceutical care and clinical reviews.
• Provide drug information to physicians, nurses, and patients.
• Oversee drug distribution and ensure formulary adherence.
• Supervise pharmacy technicians and manage daily activities.
• Coordinate discharge medications with hospital teams and hospice pharmacists.
• Participate in quality assurance, new clinical services, and policy development.
• Support adverse drug reaction reporting and patient confidentiality.

Summary

Pharmacist role involves delivering top-tier outpatient pharmacy services, ensuring the highest standards of pharmaceutical care for each patient within their designated area. The outpatient coverage includes: CWH Professional Pharmacy, where the pharmacist is responsible for patient consultations, clinical reviews, and Drug Utilization Review for dispensed medications. Tele-Pharmacy, which involves remote verification, supervising pharmacy technicians remotely, and providing on-site backup coverage as needed. Additionally, the role encompasses therapeutic assessments, interventions, and planning for both individual patients and patient populations.

Position Reports To: Pharmacy Manager

Essential Functions

1. Assure safe, effective, and cost-effective use of drugs on behalf of the patient through the application of specialized knowledge, skills and professional judgment, while considering the patient's disease conditions.
2. Maintains knowledge of age specific issues and applies that knowledge to the provision of care within our institution.
3. Assure compliance with hospital and departmental policies, procedures, and practice guidelines.
4. Provide drug information for physicians, nurses, and patients; utilizing specialized knowledge, critical thinking and available drug resources.
5. Coordination of the drug distribution system/pharmacy services for areas of responsibility.
6. Compliance with formulary management and therapeutic substitution initiatives.
7. Assure on-going improvements in therapy and systems through collaborative problem solving, research and participation in the hospital and department's organizational performance initiatives.
8. Provide direct supervision of pharmacy technicians. Responsible for overseeing and monitoring the daily activities of pharmacy technicians to appropriately maintain the provision of safe, timely and effective pharmaceuticals.
9. Assists the pharmacy manager and other staff in developing and maintaining quality assurance programs, new clinical services, an effective formulary, and appropriate policy and procedures.
10. Facilitates seamless patient care by coordinating discharge medications in collaboration with hospitalists & discharge nurses and hospice medications dispensing & delivery in collaboration with hospice pharmacists.
11. Assists in the development of protocols, guidelines and workflow.
12. Participates in the training and development of pharmacist, technicians and other health care disciplines as deemed appropriate.
13. Participate as a member of multi-disciplinary and pharmacy committees as deemed appropriate.
14. The position has independent responsibilities for organizing and planning ones work in order to accomplish daily responsibilities as well as broad goals and objectives.
15. Assist in Adverse Drug Reaction and Medication Error reporting with ideas for prevention of future incidents.
16. Maintain patient confidentiality standards.
17. Positively affects morale.
18. Ensures compliance with controlled substances policies, including inventory management, monthly counts, and daily accuracy of dispensed and on-hand counts.
19. Meets priorities as established by the Pharmacy Manager.
20. Responsible for communicating operational and personal concerns, and opportunities for improvement to department leadership.
21. Performs other duties as assigned.

Qualifications

Required:

• Bachelor's Degree (Bachelor of Science) in Pharmacy (RPh).
• Licensed to practice in the state of Washington prior to start date.
• BLS within first year of employment, and recertified bi-annually.
• Must possess basic computer skills related to Windows navigation, mouse usage, accurate keyboarding, email communication and password management.

Desired:

• PhD (Pharm. D).
• Outpatient or retail pharmacy, Hospice pharmacy and medication therapy Management.
