Job summary

PharMerica is seeking a Pharmacist for its Warwick, RI location in a closed‑door, non‑retail setting serving long‑term care and senior living clients.

You will evaluate orders, provide clinical recommendations, maintain resident profiles, and ensure regulatory compliance while collaborating with physicians and the healthcare team. The role supports medication safety and high‑quality care.

Qualifications

  • Bachelor’s degree in Pharmacy; Pharm D preferred.
  • 1 year of long-term care experience desired.
  • Current, valid pharmacist license in the state of practice.
  • Knowledge of CMS, DEA, FDA regulatory standards.
  • Proficiency with EMRs and pharmacy information systems.
  • Strong clinical judgement and communication skills.

Responsibilities

  • Evaluate medication orders for appropriateness, dosage, interactions.
  • Provide clinical recommendations to physicians and staff on therapy changes.
  • Maintain patient profiles including medical history and medications.
  • Ensure monitoring tests for high‑risk meds are ordered and reviewed.
  • Collaborate with physicians, nurses, and other professionals for clinical support.
  • Document all communications and interventions in medical records.
  • Implement policies for safe handling and management of medications.
  • Ensure compliance with regulatory requirements related to pharmacy practice.
  • Report medication errors and adverse events to the Pharmacy Manager.
  • Educate clinical staff on safe medication handling procedures.
  • Perform other pharmacy tasks as assigned.

Pharmacist – Full Time – Warwick, RI

Join our closed‑door pharmacy team where you’ll work in a non‑retail environment, dedicated to meeting the pharmaceutical needs of long‑term care and senior living clients. If you’re passionate about delivering exceptional service and high‑quality care, we want to hear from you.

Schedule

Two weekdays 1st shift 9:30am - 6:00pm; Two weekdays evening shifts 1:30pm - 10:00pm w/ every 3rd weekend

Responsibilities
  • Evaluate physician medication orders for drug appropriateness, correct dosage, potential drug interactions, and proper route of administration.
  • Provide clinical recommendations to physicians and healthcare staff regarding medication therapy changes and optimizations.
  • Maintain up‑to‑date patient/resident profiles, including medical history, lab results, and current medications (prescription, OTC, and home remedies).
  • Ensure laboratory tests required for monitoring high‑risk medications are appropriately ordered, reviewed, and communicated to the healthcare team.
  • Collaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support.
  • Document all communications and clinical interventions in the patient’s medical record, including interactions with families and caregivers.
  • Implement and enforce policies and procedures for the safe handling, dispensing, and clinical oversight/management of medications.
  • Ensure compliance with all state and federal regulatory requirements related to pharmacy practice and medication management.
  • Report medication‑related errors and adverse drug events to the Pharmacy Manager as required.
  • Educate and support clinical team and healthcare agency staff on safe and effective medication handling procedures.
  • Perform other pharmacy‑related tasks and responsibilities as assigned.
Qualifications
  • Bachelor’s Degree in Pharmacy, Pharm D preferred
  • One year experience in long‑term care desired
  • License/Certification(s): current and valid pharmacist license in the state of practice
  • Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA).
  • Proficiency in electronic medical records and pharmacy information systems
  • Strong clinical judgement, and effective written and verbal communication
