Job summary

JobRx, Inc. seeks a Pharmacist to compound and dispense prescribed drugs in a healthcare facility, transfer prescriptions, and collaborate with other clinicians to optimize therapy.

This role involves medication order entry in the computer, monitoring patient profiles, supervising support staff, and documenting interventions to ensure safe, effective patient care.

The successful candidate will meet state certifications and bring judgment and experience to planning and problem solving.

Responsibilities

• Record management: Maintain accurate medication patient profiles in the Cerner system.
• Clinical pharmacists are notified when an order is placed in the clinical queue and issues are resolved promptly.
• Consultation: Provide appropriate patient care after assessing data and needs and respond to drug information requests.
• Compounding & dispensing: Ensure medications are accurately filled, weighed, measured, and prepared under supervision.
• Ensure quality and cost effectiveness by accurate labeling and timely availability of medications.
• Participate in projects and other duties as assigned.

The Pharmacist, compounds and dispenses prescribed drugs in a healthcare facility environment, transfers prescriptions, provides drug information in collaboration to other healthcare professionals, performs medication order entry into the computer, monitors patient profiles for appropriate medication use oversees the work of the support staff and documents any relevant intervention. The position is expected to meet state certifications, requires familiarity with standard concepts, practices, and procedures, and relies on experience and judgment to plan and accomplish goals.

   2. Proficiency is developed in pharmacy information systems including the ability to troubleshoot and/or clarify orders as evidenced by the resolution of problems immediately and lack of complaints.
   3. Clinical pharmacists are notified when an order is placed in the clinical queue.
   4. Initiative is displayed to prevent, identify, and resolve pharmacy related patient care problems immediately.

   1. Appropriate adult/geriatric/pediatric/neonatal patient care is provided after assessment and interpretation of patient data and needs.
   2. Concise, applicable, comprehensive and timely responses to requests for drug information are provided to patients and health care providers as evidenced by lack of complaints.
3. Compounding & Dispensing Medication: This will have been satisfactorily performed when...
   1. Medications are correctly filled during the filling process. Supervision is provided at all times in accordance to established guidelines.
   2. Medications are accurately weighed, measured and compounded utilizing appropriate tools and equipment as evidenced by lack or errors. Supervision is provided at all times in accordance to established guidelines.
   3. Scheduled medications are correctly dispensed/returned to the Narcotic Vault 100% of the time as evidenced by reports.
4. Ensures Quality and Cost Effectiveness: This will have been satisfactorily performed when...
   1. Medications are correctly filled and accurately labeled and available to patients in a timely manner at 100% of the time.
   2. Approved Pharmacy and Therapeutics Committee substitutions are made or used when applicable at the point of order entry 100% of the time.
