Qualifications

Responsibilities

• Evaluate physician medication orders for appropriateness, dosage, interactions, and administration route.
• Provide clinical recommendations to physicians and healthcare staff regarding therapy changes.
• Maintain up‑to‑date resident profiles including medical history, labs, and meds.
• Ensure monitoring tests for high‑risk meds are ordered, reviewed, and communicated.
• Collaborate with physicians, nurses, and others to provide clinical support.
• Document communications and interventions in the patient record.
• Implement and enforce policies for safe handling, dispensing, and management of medications.
• Ensure compliance with state and federal pharmacy regulations.
• Report medication errors and adverse events to Pharmacy Manager.
• Educate clinical staff on safe medication handling procedures.
• Perform other pharmacy tasks as assigned.

PharMerica is hiring a full-time Pharmacist in Fridley, MN.

Join our closed‑door pharmacy team where you’ll work in a non‑retail environment, dedicated to meeting the pharmaceutical needs of long‑term care and senior living clients.

Schedule:

Monday – Friday: 8am to 4:30pm, 10am to 6:30pm, 11:30am to 8pm; every 5th weekend, holiday, and on‑call.
