Qualifications

  • Excellent organizational, problem-solving, and critical thinking abilities.

  • FQHC or public health experience preferred.

Responsibilities

Job Description

Reports To: Pharmacy Director

Position Summary

As a Pharmacist at Kintegra Health, you will play a vital role in delivering high-quality, patient-centered care. This position is responsible for preparing and dispensing medications, conducting comprehensive Medication Therapy Management (MTM), and providing pharmacological expertise to multidisciplinary healthcare teams. You will monitor patient drug therapies, counsel patients, collaborate with providers, and oversee pharmacy operations including inventory, personnel, and compliance. This role emphasizes clinical engagement, documentation, and a commitment to accessible, community-based healthcare.

Key Responsibilities
  • Dispensing &Operational
  • Accurately and safely fillprescriptions
  • Maintain proper filing of CII-Vprescriptions
  • Manage perpetual inventory records for CIImedications
  • Remove outdated, damaged, or recalled merchandise and prepare forreturn
  • Document and report dispensing incidents perprotocol
  • Ensure compliance with federal/state laws and internalprocedures
  • Oversee cash management and transactionaccuracy
  • Train and develop pharmacytechnicians
  • Participate in audits and maintain readiness for inspections
Clinical
  • Conduct MTM and adherence interventions using platforms such as Outcomes MTM, EQUIPP,and Aledade
  • Monitor drug regimens for safety, efficacy,andcost-effectiveness
  • Identify and resolve drug-related problems including interactions andcontraindications
  • Provide consultations to patients and healthcareproviders
  • Participate in multidisciplinary rounds and clinic teammeetings
  • Document clinical interventions in Epic and other designatedplatforms
  • Support MAP (CRA Team)and replenishment programs (Merck and DirectRelief)
  • Educate patients on medication administration, self-monitoring, and adherencestrategies
  • Collaborate with providers to optimize therapeutic outcomes and chronic diseasemanagement
  • Contribute to development of evidence-based protocols and medication safetyinitiatives
  • Serve as a resource for therapeutic alternatives and medicationmanagement
  • Conduct weekly rounding with clinical staff for collaboration andconsultation
  • Utilize certified pharmacy technicians to support clinicalworkflows
  • Precept pharmacy students and contribute to staffdevelopment
  • Provide input to Clinical Oversight and Pharmacy & Therapeutics (P&T)Committees
  • Demonstrate leadership and uphold standards of pharmacy excellence andcompliance
Minimum Qualifications
  • Doctor of Pharmacy (Pharm.D.) or Bachelor of Science inPharmacy
  • Unrestricted Pharmacy License in NorthCarolina
  • Three years of pharmacy experiencepreferred
  • Proficiency in Microsoft Office and pharmacy softwaresystems
  • Strong verbal and written communicationskills
  • Ability to work independently and collaboratively within a multidisciplinaryteam
  • Excellent organizational, problem-solving, and critical thinkingabilities

  • Prior experience in FQHC or public healthpreferred
