The selected candidate will work alongside an experienced healthcare team to support safe and effective medication management, optimize patient drug therapy, and ensure quality pharmaceutical care. This role requires strong clinical knowledge, attention to detail, and the ability to work in a hospital pharmacy environment.

Key Responsibilities

• Review, verify, and process medication orders and prescriptions for accuracy and appropriateness.
• Ensure safe and effective medication dispensing in accordance with hospital policies and regulatory requirements.
• Evaluate medication therapy for potential drug interactions, allergies, contraindications, and appropriate dosages.
• Collaborate with physicians, nurses, and other healthcare professionals regarding medication management.
• Provide clinical pharmacy support and recommendations to optimize patient outcomes.
• Monitor medication safety and identify potential medication-related issues.
• Maintain accurate medication records and documentation using the EPIC electronic medical record system.

Required Qualifications

• Current, active, and unencumbered Oregon Pharmacist License required at the time of submission.
• Minimum 1 year of relevant experience.
• Previous hospital pharmacy experience preferred.
• Healthcare Provider Level BLS certification required.
• ACLS certification required.
• PALS certification required.
• Ability to work varied shifts, including rotating weekends and holidays.
• Must successfully pass a pre-employment/post-offer drug screening.
