Job summary

RPh on the Go in Rochester, NY is seeking a Home Infusion Staff Pharmacist for a contract-to-hire opportunity. The role centers on IV therapy, sterile compounding, and comprehensive home infusion services.

You will review infusion orders, participate in clinical rounds, maintain patient records, and ensure USP and cleanroom compliance while training staff and driving operational improvements.

Qualifications

• Active NY Pharmacist license is required.
• 2+ years acute care or 1+ year home infusion experience preferred.
• Experience with IV admixture and sterile compounding desirable.

Job description

RPh on the Go is seeking a Home Infusion Staff Pharmacist for a contract opportunity in Rochester, NY. This position is ideal for an experienced pharmacist with a strong background in IV therapy, sterile compounding, and home infusion services.

Term of Assignment: Direct-Hire

Schedule: Monday – Thursday (8:00 AM – 5:00 PM)

Pharmacy system: CareTend

Job Duties:

• Review infusion orders and patient therapies
• Participate in clinical rounds
• Perform sterile compounding and TPN review
• Maintain patient records and care plans
• Ensure USP and cleanroom compliance
• Train and support pharmacy staff
• Identify and drive operational improvements
• Evaluate and monitor drug therapy based on the overall needs and clinical conditions of home care patients
• Collaborate with physicians, nurses, and other healthcare professionals to ensure appropriate patient care

Requirements:

• Current licensure as a Registered Pharmacist in the state of New York
• Minimum of two years of recent acute care experience or one year of recent home infusion therapy experience preferred
• Strong knowledge and experience with intravenous admixture services and intravenous therapy
• Ability to evaluate and monitor drug therapy in collaboration with the patient's care team
• Experience working in a sterile compounding and/or home infusion environment preferred
