UnityPoint Health’s UnityPoint Clinic – PACE is seeking a Personal Care Assistant/Certified Nursing Assistant to provide hands-on care to PACE participants in home and community settings. You will assist with ADLs, transfers, meal preparation, and document services under the PACE team.

This full-time role offers benefits, a Monday–Friday schedule with rotating weekends/holidays, and opportunities for growth within a supportive, values-driven culture.

Qualifications

  • High School Diploma or GED is required or applicable work experience.
  • CNA training certification with 75 hours of supervised training or minimum 6 months clinical experience.
  • Current CPR and Mandatory Reporter status; valid driver’s license for work-related travel.

Responsibilities

  • Assist participants with activities of daily living (ADLs) including bathing, grooming, dressing, toileting, and mobility.
  • Provide transfers, ambulation, and use of assistive devices.
  • Perform light housekeeping related to participant care (laundry, meals, cleaning).
  • Assist with meal setup and feeding as needed.
  • Accurately document services provided and report incidents promptly.
  • Follow infection control and universal precautions.
  • Participate in the PCA on-call rotation.

The Personal Care Attendant (PCA) provides hands-on personal care and supportive services to PACE participants in their homes and community settings. The PCA supports participants with activities of daily living while promoting safety, dignity, independence, and quality of life under the direction of the PACE interdisciplinary care team.

Qualifications
Education

High School Diploma, GED OR applicable work experience.Certification from a nursing assistant training program through classroom and supervised practical training totaling at least 75 hours with 16 hours devoted to supervised practical training OR minimum of six months clinical experience.

License(s)/Certification(s)

Maintains current CPR status Maintains current Mandatory Reporter status Completes 12 hours of related in-services per calendar year. Valid driver’s license when driving any vehicle for work-related reasons.Valid Auto insurance.
