Job summary

The UnityPoint Clinic- PACE program is seeking a Personal Care Assistant/Certified Nursing Assistant to provide hands-on care to PACE participants in their homes and in community settings. This role supports daily living activities and promotes safety, dignity, independence, and quality of life under an interdisciplinary care team.

Typical duties include ADL assistance, mobility support, light housekeeping, meal prep, and accurate documentation.

Qualifications

• Experience assisting with activities of daily living (ADLs).
• Nursing assistant training with supervised hours preferred.
• CPR and Mandatory Reporter status beneficial.

Responsibilities

• Assist with bathing, grooming, dressing, toileting, and mobility.
• Support transfers, ambulation, and use of assistive devices.
• Perform light housekeeping and meal preparation related to care.
• Document services provided and report incidents promptly.
• Follow infection control precautions and participate in on-call rotation.

Monday-Friday 8AM-4:30PM + rotating weekends/holidays

The Personal Care Attendant (PCA) provides hands-on personal care and supportive services to PACE participants in their homes and community settings. The PCA supports participants with activities of daily living while promoting safety, dignity, independence, and quality of life under the direction of the PACE interdisciplinary care team.

Responsibilities

• Assist participants with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, and mobility

• Provide assistance with transfers, ambulation, and use of assistive devices

• Perform light housekeeping tasks related to participant care (laundry, meal preparation, cleaning participant areas)

• Assist with meal setup and feeding as needed

• Accurately document services provided and report incidents promptly

• Follow infection control and universal precautions

• Participate in the PCA on call rotation

Qualifications

Education

High School Diploma, GED OR applicable work experience.Certification from a nursing assistant training program through classroom and supervised practical training totaling at least 75 hours with 16 hours devoted to supervised practical training OR minimum of six months clinical experience.

License(s)/Certification(s)

Maintains current CPR status Maintains current Mandatory Reporter status Completes 12 hours of related in-services per calendar year. Valid driver’s license when driving any vehicle for work-related reasons.Valid Auto insurance.
