Job summary

Kaweah Health is seeking a Home Health Aide to assist clients in their daily living activities within a home care setting in Visalia, CA. This role supports personal care, nutrition, and household tasks per the care plan.

Requires HeartSaver AED, valid California Driver's License, and completion of a CNA/MA/HHA program or LVN/RN coursework. Prior experience is preferred and a DMV report will be requested prior to offer.

Qualifications

• HeartSaver AED certification is required.
• Valid California Driver's License; DMV report required before offer.
• Education: Certified Nursing Assistant, Medical Assistant, Home Health Aide, or LVN/RN (one semester) in recent past.

Responsibilities

• Assist client with personal care per care plan; help with toilet needs, ambulation, and transfers.
• Support nutrition: plan meals, shop for groceries, prepare and serve food per diet.
• Provide household services to maintain a safe, hygienic environment.
• Perform procedures within scope of practice after formal training and supervisor guidance.
• Report changes in condition to supervisor.
• Maintain continuity of care with the home care team.
• Deliver compassionate care to patients nearing end of life.
• Demonstrate knowledge of age/cultural needs and communicate with patients/families accordingly.
• Perform other duties as assigned.

QUALIFICATIONS

• License /Certification
  Required:
• HeartSaver AED
  Valid California Drivers License. Must provide DMV report prior to offer being made. No driving infractions 5 years prior to hire or during time in this job. Infractions include DUI, Suspended or Revoked License, Reckless Driving
• Education
  Required: Completion of a Certified Nursing Assistant class, Medical Assistant class, Home Health Aide class, or one semester of LVN or RN in recent past.
• Experience
  Preferred: Previous experience preferred.

JOB RESPONSIBILITIES

• Assists client/patient with personal care according to the care plan to achieve established goals. Assists with toilet needs and ambulation and transfer of the patient/client.
• Supports the patient/client’s nutrition through meal planning, purchasing groceries, preparing and serving food according to dietary needs.
• Provides household services deemed necessary to maintain a safe, hygienic environment.
• Performs special procedures when formally trained to do so, under the direction and authority of the supervisor, within scope of practice.
• Reports pertinent information, including changes in condition/problems to the supervisor.
• Maintains continuity of care through the support of the home care team.
• Provides exceptional level of care with compassion and sensitivity to those nearing the end of life’s journey.
• Demonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area. Knowledgeable of growth and development for all patient/family cultural, linguistic, spiritual, gender, and age specific needs. Able to effectively communicate and care for patient and family as reflected in the Plan for Provision of Care.
• Performs other duties as assigned.
