Job summary

VTSS is hiring a Permanency Coordinator to provide short-term support and stabilization services for youth and families across Barre, Montpelier, Springfield and surrounding counties in Vermont. The role involves coordinating treatment, discharge planning, and case management with regular community travel and collaboration with a multidisciplinary team.

Qualifications

• Preferred: Bachelor’s degree in human services or therapeutic services.
• Minimum: Associate’s degree in human services or therapeutic services.
• Valid driver’s license with active car insurance.
• Satisfactory background checks and licensing checks.

Responsibilities

• Travel an average of 8+ hours weekly within Vermont to family homes.
• Coordinate treatment and discharge planning for permanency goals.
• Perform case management and community integration tasks.
• Document services and participate in weekly team meetings.
