Responsibilities

• Assess physical, psychological and social dimensions; develop care plan.
• Document nursing interventions and patient responses; summarize data on discharge.
• Discharge planning and patient/family education.
• Coordinate unit routines and supervise staff; orient new personnel.
• Communicate plan via bedside shift report and provide clear instructions.
