The role involves teaching skills for independence, promoting community inclusion, assisting with daily living activities, mentoring social skills, and documenting daily activities per the Individual Service Plan.

We are hiring compassionate, hard working, friendly and reliable AFL providers who will work with individuals with intellectual and/ or developmental disabilities. (I/DD) If you don't have previous I/DD experience, we'll provide training!
Primary Purpose of Provider: This position provides support and care for the consumer living in the AFL provider's home. The AFL Provider implements activities as outlined in the Individual Service Plan prepared. All activities are documented daily.

The AFL provider supports the individual or individuals 24 hours a day, 7 days a week, unless the consumer is involved in other day programs or has been approved to receive respite or visit with natural support.
