Community of Hope seeks a temporary Perinatal Services Program Supervisor to oversee perinatal education, care navigation, and support services across multiple clinical sites in Washington, DC. You will supervise up to four staff and report to the Director of Perinatal Services.

This on-site role at Conway Health & Resource Center in SW DC has an anticipated end date of 3/31/2027, with a compensation range of $31.25–$36.05 per hour and a commitment to equitable, high-quality care.

Qualifications

• Bachelor's degree in social work or education or equivalent experience.
• Experience providing social services, care coordination or health care education.
• Ability to multi-task and problem solve.
• Ability to travel between sites and respond to urgent staff or patient needs during off hours.
• Proof of required vaccinations is required.

Community of Hope is seeking a temporary Perinatal Services Program Supervisor to provide strategic oversight and day-to-day leadership for the delivery and integration of perinatal education, care navigation, and support services across multiple clinical sites. In this role, you'll ensure that perinatal patients have timely access to high-quality education, resources, and evidence-based interventions designed to improve maternal and child health outcomes. This role will report to our Director of Perinatal Services, and will be responsible for the supervision of up to four Perinatal Services direct service staff members.

This temporary position is located at our Conway Health & Resource Center in SW, Washington, DC., with an anticipated end date of 3/31/2027.

What You'll Do

• Ensures that all prenatal patients are adequately assessed at the start of their prenatal care using required tools (screeners, intake questionnaires, OB Auths) and are connected to the appropriate care coordination services programming timely.
• Ensures all perinatal education and navigation recipients receive evidence-based interventions, appropriately timed screeners, and health education to positively influence behaviors that impact maternal and child outcomes. Examples of evidence-based interventions and health education to be provided via the education and navigation team includes smoking cessation, substance use cessation, lactation, early entry care, consistent attendance of appointments and specialty referrals, and safe sleep practices.
• Revises existing and develops new perinatal and infant care education materials in collaboration with Maternal and Child Health (MCH) and clinical leadership to support educational interventions provided by MCH staff and ensure all education provided is current and culturally congruent.
• Develops promotional activities of group offerings, maintains timely posted calendars and group descriptions on COH website and social media outlets, ensures that accurate and representative flyers are designed and available for timely distribution.
• Establishes program outcome targets and ensures the alignment of targets with grant responsibilities. Manages and analyzes these targets by contributing data monthly to the Child and Maternal health data dashboard and generating monthly narrative reports for internal and grant reporting purposes.
• Monitors program outcomes, reviews caseload distributions, and collects staff feedback to optimize program delivery by developing, standardizing, and memorializing workflows. Ensures workflows adequately communicated across relevant care team members.
• Identifies gaps in services to address social determinants of health based on outcomes and patient feedback. Assists in developing plans, community resources and other partnerships to meet identified needs. Shares resources on external social service agencies to meet needs of patients for housing, legal, domestic violence, substance abuse, etc.
• Develops and manages relationships with community organizations to provide support and resource services for patients, including developing and managing memorandums of understanding or contracts; contributing to presentations or written reports on programming.
• Manages budget for programmatic activities, approving expenditures in a timely fashion, generating spending plans. Ensures that grant funds are utilized in accordance with grant or contract requirements and timelines.
• Attends and contributes in program, departmental, clinical, and leadership meetings to support integration of programming by identifying relevant connection points, advocating for standardization that positively impact program’s outcomes, highlighting needs of programs and staff, and volunteering to participate in opportunities that give recruitment opportunities and/or build program staff’s capacity to serve their target population and meet program goals.

Must-Haves

• B.A./B.S. in social work or education or equivalent experience required.
• Experience providing social services, care coordination or health care education required.
• Ability to multi-task and problem solve required.

• Ability to travel between sites required. Ability to respond to urgent staff or patient needs during off hours required.

Nice-to-Haves

• Experience in community health center setting preferred.
• Relevant supervisory background preferred, minimum two years.
• Bilingual preferred in English and Spanish.
