Job Purpose

Healthier Moms and Babies (HMB) is seeking a Registered Nurse to support pregnant and postpartum women affected by substance use disorder (SUD) and their infants through a specialized home-visiting model. This position will provide clinical care coordination, recovery-oriented support, and maternal–infant health education from pregnancy through the postpartum period, with a focus on preventing maternal overdose, improving recovery engagement, and supporting healthy infant development.

The nurse will deliver trauma-informed home visits, conduct screening and brief interventions using Screening, Brief Intervention, and Referral to Treatment (SBIRT) principles, and coordinate care with obstetric providers, behavioral health providers, and community partners. The nurse will complete NAADAC Women in Recovery Specialty Training, which focuses on issues specific to women in substance use recovery, including perinatal substance use disorders, co-occurring mood disorders, culturally responsive care, and long-term wellbeing in recovery, and integrate these gender-responsive, recovery-oriented strategies into nursing practice.

This role is central to HMB’s efforts to reduce maternal mortality, improve pregnancy outcomes, and support infants affected by prenatal substance exposure.

Responsibilities

Maternal Health and Recovery Support

• Conduct regular home visits with pregnant and postpartum women experiencing current or prior substance use disorder.
• Provide trauma-informed education on maternal health, recovery, overdose prevention, and postpartum wellbeing.
• Deliver Screening, Brief Intervention, and Referral to Treatment (SBIRT) for substance use, perinatal mood disorders, and social determinants of health.
• Support treatment engagement and continuity of care for substance use and mental health services during pregnancy and through the postpartum period.
• Reinforce treatment and recovery plans developed by behavioral health and medical providers.

Care Coordination and Referrals

• Coordinate care with obstetric providers and other clinical partners.
• Facilitate warm handoffs to substance use treatment, behavioral health providers, and community-based recovery supports.
• Address barriers to care including transportation, childcare, scheduling challenges, and navigating healthcare and social service systems.
• Maintain communication with healthcare providers and community partners to ensure coordinated care.

Infant Health and Development

• Provide caregiver education on infant health, development, and the effects of prenatal substance exposure.
• Monitor infant health and developmental milestones and support adherence to well-child visits and recommended screenings.
• Reinforce pediatric and specialty care recommendations for infants affected by Neonatal Abstinence Syndrome (NAS).
• Promote healthy attachment and strengthen the mother–infant relationship through dyadic, family-centered interventions.
• Identify and address social needs impacting recovery and family stability, including housing, employment, childcare, and transportation.
• Connect families with community resources and assist in accessing services.
• Collaborate with HMB staff and community organizations to ensure comprehensive support for participating families.

Program Documentation and Evaluation

• Maintain accurate documentation of home visits, screenings, referrals, and care coordination activities in HMB’s data systems.
• Track participant engagement, outcomes, and service delivery in accordance with grant and program requirements.
• Participate in case reviews, team meetings, and continuous quality improvement activities.

Professional Development

• Complete NAADAC Women in Recovery Specialty Training within the first 3-6 months of employment.
• Complete Community Health Worker Training within 6 months of employment.
• Participate in ongoing professional development related to substance use disorder, trauma-informed care, maternal mental health, and infant development.
• Remain current on best practices in maternal health, recovery support, and neonatal abstinence syndrome.

Qualifications

Education
