Job summary

Deschutes County Health Services is seeking a Public Health Educator I – Community Health Worker for the Perinatal Care Coordination program within the Family Support Services team in Bend. The nurse home visiting programs include Babies First!, CaCoon, and Perinatal Care Coordination (PCC).

You will provide PCC services, referral coordination, and health promotion and education tailored to perinatal populations in our community.

Qualifications

• Experience in public health education or health promotion.
• Experience coordinating care and referrals.
• Ability to work with community partners and clients.

Responsibilities

• Conduct clinic and telehealth visits with clients and connect them with resources.
• Provide service navigation, care coordination, and referrals to community resources.
• Document visits and care plans in the EHR and data systems.
• Build relationships with community partners and promote perinatal services.
• Manage referrals for perinatal services within Deschutes County Health Services.
