Job summary

CentraState Healthcare System in Freehold, NJ, is seeking a Per Diem Registered Nurse for the Wound Care Center. The role supports day-to-day operations, aids physicians, and coordinates patient care throughout the center.

The nurse will gather data, participate in performance improvement initiatives, and help develop center protocols while maintaining high-quality patient care. Knowledge of wound care and 1 year acute care RN experience preferred.

Qualifications

• Associate’s degree in Nursing is required.
• Bachelor’s degree in Nursing within 3 years of hire.
• Current NJ RN license and BLS certification required.
• Minimum of 1 year acute care RN experience required.
• Wound Care experience preferred; progress toward Wound Certification within one year of hire.

Responsibilities

• Reviews visits for appropriate patients who would benefit from referral to Hyperbaric/Wound Care programs.
• Collects data on patient visits and follow ups to maintain department statistics.
• Answers and provides clinical information to patients and the community via phone, in person, and other channels.
• Establishes and maintains insurance matrix for participating physician offices.
• Demonstrates professional networking with hospital staff, doctors and other healthcare professionals.
• Assists in developing protocols, policies and promotes center services.
• Coordinates services with other health care personnel.
• Provides education to patient/family regarding disease processes and planned treatment/intervention.
• Coordinates workflow and patient assignments in the department.
• Demonstrates autonomy and ability to work independently.
• Assists in coordinating department education/in-service schedule and symposium schedule.
• Evaluates patient progress and need for further services following discharge.
• Maintains department inventory of supplies and orders supplies when needed.
• Tracks patients throughout system to ensure high quality and expedite care paths.
• Maintains productivity within guidelines set by department.
• Cleans rooms between patient use.
• Follows standard precautions, isolation precautions, hand hygiene, and proper PPE use.
• Supports operations by floating between units as an RN and providing 1:1 patient observation when needed.
