BAYADA Home Health Care is seeking an experienced Speech Language Pathologist, SLP/ST, for a per diem home health opportunity in Forsyth County, NC. The SLP will provide evaluations and therapy to adults and seniors in patients' homes across designated counties.

The role requires a current NC SLP license and ASHA CCC-SLP or equivalent, with EMR documentation and coordination with a multidisciplinary team. Per diem scheduling supports flexible hours and professional growth.

Formación

• Current North Carolina SLP license or eligibility for licensure.
• CCC-SLP or equivalent certification by ASHA, or be eligible for certification.
• Process to obtain licensure or certification while employed.

Responsabilidades

• Make home visits to clients in designated NC territories.
• Evaluate and treat adults and geriatric clients with speech, language, or swallowing disorders.
• Develop and adjust treatment plans based on evaluations and physician orders.
• Collaborate with multi-disciplinary team to plan patient care.
• Educate patients and families on rehabilitative care and safety.
• Document observations and interventions in EMR.
