Apple Rehab seeks a Per Diem Social Worker to support the Director of Social Services during maternity leave at Wolcott Hall Nursing Center in Torrington, CT. This role follows residents through admission and transition, ensuring social/emotional needs are met and coordinated within the care continuum.

Responsibilities include documenting plans of care, collaborating with nursing, MDS, and rehabilitation teams, and educating staff and families about residents' rights.

Qualifikationen

• Bachelor's or Master's Degree in Social Work or related field.
• Experience in a long-term care facility and familiarity with Requirements of Participation regulations is preferred.
• Knowledge of admissions and discharge procedures and residents’ rights and documentation requirements.
• Attention to detail, good follow through, and ability to prioritize multiple tasks.
• Innovative and able to find creative ways to meet residents' needs.
• Must have a vehicle and a valid driver's license.
• Demonstrates cooperation and ability to work as a team.
• Patience, tact, enthusiasm and a positive attitude toward residents and their support networks.
• Ability to remain calm in stressful situations.
• Ability to communicate with residents, staff and families.

Aufgaben

• Maintain a plan of care for residents' social/emotional needs in the EMR.
• Collaborate with nursing, MDS and rehabilitation to facilitate discharge plans.
• Inform and educate staff, residents, families about residents' rights and health services.
• Maintain resources for residents and families regarding community services and agencies.
• Provide timely referrals to appropriate agencies (e.g., Medicaid).
