Job summary

At Home Healthcare, a part of LHC Group, a leading post-acute care partner for hospitals, physicians and families nationwide, invites you to join our team as a Social Worker - Per Diem. You’ll provide medical social services under the direction of a physician and an Interdisciplinary Group to understand social and emotional factors related to the patient’s health status and help develop coping mechanisms.

Pay ranges from $35.10 to $52.65 per hour, with full-time considerations.

Qualifications

• Master's Degree from a school of Social Work accredited by the Council on Social Work
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• Licensed as a Social Worker from the State Board of Social Work
• Current CPR certification or ability to complete within 90 days of hire
• Licensed Social Worker in the state of residence
• 1+ years of social work experience in a health care setting
