Job summary

Harbors Home Health & Hospice is seeking Per Diem RNs and LPNs in Washington state to deliver individualized home health and hospice care. You’ll work in a supportive, team-oriented environment with flexible scheduling to fit life’s priorities.

Our clinicians help develop patient care plans and have opportunities for advancement, mileage reimbursement, and modern flexibility while providing high-quality care to communities in Grays Harbor and Pacific Counties.

Qualifications

• Current WA State Nursing License (RN or LPN).
• CPR/BLS Certification required.
• Valid driver’s license & dependable, insured automobile.

Responsibilities

• Provide individualized, one-on-one home health and hospice care.
• Develop and implement patient care plans with flexibility to schedule around life commitments.
• Collaborate with the care team to ensure quality, compassionate patient outcomes.
