Job Description

The Nurse Practitioner is dedicated to enhancing the health and well‑being of patients/families, nurses and the healthcare system by positively influencing patient outcomes utilizing evidence based practice and promoting excellent bedside practice through collaboration, leadership and mentoring.

Qualifications
  • CA Nurse Practitioner (NP) and Nurse Practitioner Furnishing Number
  • DEA registration
  • CA RN license
  • BLS

Preferred: National Nurse Practitioner certification from American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC); for Hospice Advanced Practice Certified Hospice and Palliative Nurse (ACHPN).

Education – Preferred: MSN or DNP

  • One year of recent RN specialty experience in an adult, critical care, or medical‑surgical setting.
  • For Hospice: two or more years of palliative care experience with patients at end of life.
  • One year of recent practice as a Nurse Practitioner with an emphasis in adult assessment and management of medical and/or surgical conditions.

Knowledge/Skills/Abilities

  • Must complete credentialing through Medical Staff Interdisciplinary Practice Committee.
  • Excellent written and oral communication skills and effective interpersonal relationships.

Job Responsibilities
  • Responsible for enhancing Patient Throughput throughout the Medical Center and Chronic Disease Management Clinic.
  • Use knowledge and assessments of patients to design, implement and evaluate plans of patient care related to specialty area.
  • Manage populations with disease states and non‑disease based etiologies to improve and promote health care outcomes.
  • Collaborate with interdisciplinary team on most complex patients within specialty area on a consistent basis.
  • Focus on population health management across the care continuum.
  • Facilitate problem‑solving across medical staff, nursing unit and ancillary departments.
  • Design, implement, and evaluate innovative individual and/or population‑based programs of care to achieve high quality, cost‑effective outcomes.
  • Provide relevant patient instruction and education.
  • Coordinate patient care including ordering, interpreting, and evaluating laboratory tests and radiographic studies to expedite treatment; follow up on all abnormal findings.
  • Order transfers within the hospital or to other facilities to the appropriate level of care.
  • Consult with medical staff, residents, nursing, case management, social work and clinical documentation staff as needed to meet documentation and throughput goals.
  • Complete history and physical or medical screening exam, discharge summary/instructions and provide patient prescriptions as indicated to enhance throughput.
  • Document necessary components in the medical record to meet documentation standards for the assigned DRG or APR‑DRG, including appropriate patient status and level of care.
  • Document and order necessary components to meet CMS, core measure and Value‑Based purchasing requirements.
  • Serve as a leader/consultant/mentor/change agent in advancing the practice of nursing among other nurses and across organizations to achieve outcomes.
  • Utilize direct clinical practice to consult, educate, coach, and mentor staff nurses in achieving optimum patient outcomes.
  • Develop high‑quality health care standards, assist in implementation of standards, order sets, clinical pathways, and facilitate goal setting while measuring achievements, and evaluating outcomes.
  • Advance nursing practice through innovative evidence‑based interventions, best‑practice guidelines, and modification of professional standards and organization policies to improve outcomes.
  • Use clinical inquiry and research in an advanced specialty area of practice.
  • Promote use and understanding of evidence based practice.
  • Review standards of practice and national guidelines for specialty area to determine appropriateness and to reflect current nursing clinical practice.
  • Lead multidisciplinary groups to facilitate collaboration with other disciplines in the attainment of outcomes across the continuum of care.
  • Perform consultative functions; provide clinical expertise and make recommendations to physicians, other health care providers, patients, and health care organizations.
  • Attend appropriate Medical staff committees.
  • Complete and maintain appropriate procedural training for the population being served (i.e., Cardiac Stress Testing).
Addendum (essential for specific dept)
  • Cardiac Catheterization Lab responsibilities: coordinate overall clinical program and ensure processes for smooth operations and patient flow for cardiology services specific to heart valve devices (TAVR); develop and implement required elements of an effective clinical program; develop methods to ensure prompt access to care; ensure processes for coordinated patient flow across physicians clinical sites; develop collaborative partnerships with inpatient and outpatient facilities implementing the valve device; develop and implement clinical guidelines; align EMR to support care delivery; facilitate establishment of clinical trial protocols at all sites in partnership with the Valve Consortium; provide program and clinical input to marketing team to ensure web and related marketing tools are up to date; oversee practice development; direct development of education materials; develop educational materials and training program for all patient care staff; help develop valve program at affiliated hospitals that provide a referral base of patients; provide program and clinical input to quality assurance and quality check teams to ensure tools are up to date.
  • Hospice responsibilities: understand Hospice Conditions of Participation related to RNPP regulations; perform timely hospice face‑to‑face visits including timely, accurate documentation.
Additional competencies

Demonstrate knowledge of Joint Commission standards, California’s Nursing Practice Act, applicable sections of California Title 22 and 16 and all other applicable regulations, applicable Federal guidelines and regulations including HIPAA, American Nurses Association Scope and Standards of Practice, policies and procedures for specialty area, national standards for specialty area; demonstrate professional citizenship and fiscal responsibility in the healthcare system; demonstrate knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area; knowledgeable of growth and development for all patient/family cultural, linguistic, spiritual, gender, and age specific needs; effectively communicate and care for patient and family as reflected in the Plan for Provision of Care; perform other duties as assigned.
