Job summary

Best Home Health Providers Inc. is seeking a per diem Registered Nurse to ensure accurate clinical documentation and high-quality home health care across the Bay Area and beyond.

You will review OASIS assessments, code diagnoses with ICD-10, and collaborate with clinical staff to maintain data integrity and regulatory compliance. The role requires 1–2 years in home health, strong communication, proficiency with EHR systems, and a valid California RN license in good standing.

Qualifications

• Active Californian RN license in good standing.
• 1–2 years of clinical home health experience.
• Proficiency with OASIS assessments and ICD-10 coding.
• Strong communication and attention to detail.
• Proficient with computer systems and electronic health records.

Responsibilities

• Review OASIS assessments for completeness, appropriateness, and regulatory compliance.
• Ensure accurate ICD-10 coding and sequencing.
• Collaborate with clinical staff to resolve documentation and data integrity issues.
• Monitor visit utilization and flag potential financial risks or underutilization.
• Identify trends impacting agency outcomes and quality measures.
• Conduct OASIS inter-rater reliability visits with clinical team members.
• Participate in Quality Improvement and Corporate Compliance activities.
• Stay current with best practices through ongoing education.
