Job summary

Gurwin Healthcare System in Commack, NY is seeking a part-time Registered Nurse for our Licensed Home Care Agency (LHCSA) to visit patients at home and supervise certified Home Health Aides.

A New York State Nursing License is required; a BSN is preferred; prior home care experience is required; travel to patient homes and using a laptop/tablet is expected. Responsibilities include visits, assessments, supervision, documentation, and care coordination.

Qualifications

• A current New York State Nursing License.
• A Baccalaureate Degree in nursing is preferred.
• Previous home care experience is required.
• Comfortable with laptop/tablet and integrating them into a home visit.

Responsibilities

• Driving to visit patients in their homes.
• Assessments and continuing care visits to patients in the LHCSA program.
• Supervision of home health aides including instructions for implementing written plans of care.
• Notifying the agency staff of any change in the patient's condition.
• Documentation of visits.
• Other nursing services as needed.
