Qualifications

• Current license as a Registered Nurse in NJ.
• Licensed driver with automobile insured according to state requirements.
• Experience in performing initial assessments and developing care plans.

Responsibilities

• Perform comprehensive initial assessments of client status.
• Develop individualized plans of care with client input.
• Conduct client reassessments and maintain compliant records.
• Provide training and support to caregivers.

Skills

Current CPR certification

Good driving record

Ability to develop individualized care plans

Education

Graduate of an accredited school of professional nursing

BSN, PHN preferred
