Job summary

Parx Home Health Care is seeking a Registered Nurse to join our team and provide high-quality care in patients' homes and communities.

The ideal candidate will have strong clinical skills, excellent communication, and experience with electronic health records. An unrestricted RN license and 1–3 years in home health, hospice, or community health are preferred. Commitment to compassionate, patient-centered care is essential.

Qualifications

• Current and unrestricted RN license in the state of Florida.
• 1-3 years of clinical experience in home health, hospice, or community health.
• Excellent communication and interpersonal skills.
• Proficient in using electronic health records and other healthcare technologies.

Responsibilities

• Conduct comprehensive assessments of patients’ health status, including physical, emotional, and social factors.
• Develop and implement individualized care plans tailored to each patient’s needs and goals.
• Provide patient education and support to promote self-management and prevent complications.
• Coordinate care with other healthcare providers, including physicians, therapists, and social workers, to ensure continuity of care.
• Administer medications, treatments, and procedures as ordered by the physician or authorized healthcare provider.
• Document all patient care activities and communicate patient status and progress to the healthcare team.
• Participate in quality improvement activities and initiatives to improve patient outcomes and satisfaction.
• Maintain professional knowledge and skills by attending continuing education programs and staying up to date with the latest developments in the field.
• Provide emotional support and compassionate care to patients and their families.
• Adhere to all applicable laws, regulations, and ethical standards of nursing practice
