Job summary

Qualifications

• Bachelor's degree in Nursing is required.
• Valid, unrestricted Florida RN license required.
• Case Management certification (CCM or ACM) preferred.

Responsibilities

• Assess complex patient care needs and data from assessments.
• Coordinate patient care across episodic care using InterQual or Milliman criteria.
• Complete electronic documentation for discharge planning and throughput.
• Present care plans and outcomes at rounds and with leadership.
