Job summary

Community Wellness Partners in Clinton, NY is seeking a licensed Physical Therapist Assistant to provide supervised physical therapy services under the direction of a licensed Physical Therapist. The position requires NYS licensure and at least two years of relevant experience.

You will implement prescribed therapy plans, assist with patient transfers and ambulation, document progress, and communicate with nursing staff to coordinate care.

Qualifications

• Must have successfully completed the New York State licensing exam.
• A minimum of two years experience.

Responsibilities

• Administers physical therapy to patients while under the supervision of and as assistant to the physical therapist.
• Instructs, motivates and assists patients in performance of exercise programs, transfer and ambulation techniques.
• Observes patient responses to therapy and reports orally or in writing to the physical therapist.
• Reports patient responses to therapy and/or status to Home staff when necessary.
• Writes progress notes on patients designated by the RPT; notes to be co‑signed by registered therapist.
• Completes team conference forms and MDS+ as needed; attends team conference and serves as chairperson when indicated.
• Provides in‑service training as requested.
• Compiles monthly statistical data on patient visits and treatments; obtains physician orders on evaluations, files evaluations and keeps records of patient status.
• In the absence of the physical therapist, supervises all operations of the department and directs the physical therapy aides.
• Performs other duties as requested.

Reports to: Registered Physical Therapist, Director of Nursing Services and/or Executive Director in absence of RPT

Physical Demands

Stands, walks most of the day. Works in reaching or strained positions most of the day. Pushes, pulls, moves and lifts residents most of the day.

Working Conditions

Daily exposure to soiled linen and occasionally body waste.

Possible injury to self (back strain).

Will follow safe work practices in accordance with facility health and safety policies.
