Job summary

STAR Physical Therapy is seeking a motivated Physical Therapist for per diem home health around Nashville, with optional outpatient clinic hours. This 1099 role offers flexibility, autonomy, and the ability to schedule visits across the week while complementing other positions.

New graduates are encouraged to apply, and mentors help with documentation and compliance as needed in a growing outpatient/home health setting.

Qualifications

• Physical Therapist Program Graduate.
• TN Physical Therapist License or eligible.
• Liability insurance is required.
• CPR/First Aid certification required.

Responsibilities

• Accept and manage patients within your designated geographical area of coverage.
• Plan, prepare, and implement customized therapy programs designed to restore overall function in the home setting.
• Develop and maintain individualized care plans/goals in collaboration with physicians, patients, and their families.
• Ensure timely and accurate documentation of all patient visits and therapy services.
• Effectively communicate discharge planning with patients, families, and interdisciplinary team members.
• Maintain compliance with all clinical and documentation standards.

Education

Physical Therapist Program Graduate
