Job summary

Brigham and Women's Faulkner Hospital, Inc. is seeking a per diem Social Worker to evaluate patients' psychiatric conditions and assess needs as part of an interdisciplinary team.

You will develop and implement treatment plans focused on patient-centered care and arrange discharge planning and post-hospital follow-up when appropriate. The role involves daily patient contact, leading weekly social work groups as needed, family outreach, and collaboration with outpatient teams.

Qualifications

• Master's of Social Work (MSW) from an accredited program required.
• Massachusetts LICSW or LCSW with relevant clinical experience required.
• Graduate level internship experience in relevant setting.

Responsibilities

• Individual psychiatric evaluations.
• Daily individual contact with assigned patients.
• Leading weekly social work treatment groups as needed.
• Family outreach, counseling and psychoeducation.
• Coordination with outpatient treatment team.
• Arrange discharge planning needs and follow up.
• Prepare Treatment Plans and Discharge Summaries to meet requirements.
• Collaborate with other disciplines and participate in committees.
