Qualifications

• Bachelor’s, Master’s, or Doctorate required; if a Bachelor’s degree is held, 1 year of prior experience is required.
• Current New York State license or a limited permit as a Registered Physical Therapist.
• If a New York State license or limited permit is not held, must be hired within 90 days of graduation and meet Practice Exemption under NYS Physical Therapy License requirements.
• If working in home care or traveling between locations, NYS license required.

Responsibilities

• Conducts comprehensive physical therapy evaluations and develops individualized treatment plans to restore function, manage pain, and prevent disability.
• Administers and adjusts therapy interventions, provides patient and caregiver education, oversees assistants, and ensures smooth discharge planning and continuity of care.
• Collaborates with healthcare providers, participates in quality improvement and ongoing professional development, and completes all required documentation.
