Position Summary

We have an exciting opportunity to join our team as a Per Diem Physical Therapist – Neuro – Patchogue. In this role, the successful candidate will provide appropriate therapy to promote the improvement of mobility, functional ability, and quality of life. The candidate will use clinical expertise to plan the delivery of safe, therapeutic, and age‑appropriate care through examination, evaluation, diagnosis, and physical intervention.

Job Responsibilities

• Perform objective functional evaluations with appropriate tests and measures and establish a plan of care with measurable and functional goals.
• Implement appropriate interventions and treatments as indicated by the patient’s functional status, diagnosis, and age in conjunction with the plan of care.
• Maintain accurate, complete, and timely documentation reflecting examination, evaluation, diagnosis, prognosis, plan of care, education, and interventions.
• Interact and collaborate with other healthcare team members to optimize patient outcomes and health care goals. Provide on‑site clinical supervision to Physical Therapist Assistants and Rehabilitation Aides.
• Evaluate patient progress toward functional goals and modify the plan of care as appropriate.
• Provide patient, family, and clinician education regarding the patient’s functional status and plan of care.
• Perform other duties as assigned.

Minimum Qualifications

• Bachelor’s Degree in Physical Therapy from a CAPTE‑approved PT program.
• Current and valid NYS Physical Therapy license and registration or limited permit.
• Current and valid BLS or CPR certification from the American Heart Association – Inpatient facility only.
• Excellent oral and written communication skills.
• Basic computer skills.

Preferred Qualifications

• Master’s Degree in Physical Therapy from a CAPTE‑approved PT program.
• Doctorate in Physical Therapy (DPT) from a CAPTE‑approved PT program.
• Current and valid BLS or CPR certification from the American Heart Association – Outpatient facility only.
• 1–2 years experience in a Hospital (Inpatient) or Orthopedic (Outpatient) Rehabilitation setting.
• Experience in a Neuro – Outpatient facility only.
