Job summary

Always Compassionate Health is seeking dedicated, patient-focused Physical Therapists for a per diem role in Nassau & Suffolk Counties, Long Island, NY. You will evaluate patients, design individualized treatment plans, and guide mobility rehabilitation to improve independence.

Applicants must hold an active New York State PT license and a degree from an accredited PT program, with strong clinical and communication skills and the ability to work independently or in a team.

Qualifications

• Active New York State Physical Therapist license required.
• Degree from an accredited Physical Therapy program.
• Strong clinical, communication, and interpersonal skills.
• Previous experience preferred.
• Ability to work independently and as part of a team.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide therapeutic exercises, gait training, balance training, and mobility rehabilitation.
• Monitor progress and adjust treatment plans as needed.
• Educate patients and caregivers on home exercise programs and safe mobility.
• Maintain accurate clinical documentation and collaborate with healthcare professionals.
