Job summary

Constellation Home Care is seeking a PER DIEM Physical Therapist (PT) for our home health program. This field-based role requires partnering with patients to restore motion and maximize functional outcomes.

You will manage the PT plan of care, educate patients and caregivers, and document in the EMR system. Travel to patient homes and excellent training in-house are provided to support your success.

Qualifications

• Licensed Physical Therapist in the state of CT
• 1–2 years clinical home care experience preferred (new grads considered)
• Oasis experience strongly desired
• Reliable transportation required to travel to patients’ homes
• Basic knowledge of iOS operating system

Responsibilities

• Initiates, reviews, and implements the physical therapy plan of care for each patient based on the initial assessment and prescribed plan of care.
• Educate patients and caregivers on disease processes, medications, plan of care and individualized treatment plan.
• Maintains and processes all documentation in compliance with agency standards using the EMR system.
• Submits all clinical documentation, time records and other information in the required time frame.
