Qualifications

• Graduate of an accredited Physical Therapy Program.
• Current New York State PT License.
• Nursing Home experience preferred.

Responsibilities

• Evaluate residents and develop appropriate physical rehabilitation plans.
• Supervise the activities of physical therapy assistants.
• Help residents improve mobility and relieve pain through exercises and manual therapy.
• Document services in patients' medical records and communicate with families and care team.
• Submit billing logs and necessary information on a timely basis.
