Qualifications

• Doctorate or Master's degree in Physical Therapy from an accredited program.
• Current and valid Physical Therapist license in the state of practice.
• Current CPR certification (preferred or required in accordance with organizational policy).
• Strong clinical, documentation, organizational, and communication skills.
• Proficiency in mobility and gait assessments, balance and postural control interventions, adaptive equipment recommendations, and EHR systems.
• Valid driver's license, clean driving record, reliable transportation, and willingness to travel to home and community-based service locations.
• Experience supporting individuals with Intellectual and Developmental Disabilities (IDD) and providing PT in home/community settings preferred.
• English proficiency required; bilingual (including Spanish) a plus.

Job Description

OneWell Health Care is seeking a compassionate Physical Therapist (PT) to provide personalized therapy to individuals with Intellectual and Developmental Disabilities (IDD) in home and community-based settings in Pennsylvania.

You will evaluate each individual's physical needs, develop goal-based treatment plans, and collaborate with caregivers and other professionals to improve mobility, strength, balance, safety, and overall independence.

What We're Looking For:

• Doctorate or Master's degree in Physical Therapy from an accredited program.
• Current and valid Physical Therapist license in the state of practice.
• Current CPR certification (preferred or required in accordance with organizational policy).
• Strong clinical, documentation, organizational, and communication skills.
• Proficiency in functional mobility and gait assessments, balance and postural control interventions, adaptive and assistive equipment recommendations, evidence-based physical therapy techniques, and electronic health record (EHR) systems.
• Valid driver's license, a clean driving record, reliable transportation, and willingness to travel to home and community-based service locations. Flexibility to support varying schedules and assigned service areas is required.
• Experience supporting individuals with Intellectual and Developmental Disabilities (IDD) and providing physical therapy services in home or community-based settings is preferred.
• English proficiency is required. Bilingual proficiency (including Spanish) is a plus.
