Job summary

Honor Health Network is seeking a compassionate per diem Occupational Therapist to perform home visits in Norwalk, CT and surrounding areas. You will assess patients, develop personalized treatment plans, and educate on exercises and injury prevention while collaborating with the healthcare team.

Qualifications

• Licensed OT in Connecticut is required.
• Excellent communication and interpersonal skills.
• Ability to develop and implement individualized treatment plans.

Responsibilities

• Evaluate patients' physical conditions and create personalized treatment plans.
• Develop and implement therapeutic exercises to improve mobility, strength, and function.
• Monitor patient progress and adjust treatment plans as needed.
• Educate patients and families on exercises, safety techniques, and home care.
• Collaborate with healthcare teams to ensure comprehensive care.
• Maintain accurate and up-to-date patient records.
• Provide guidance on injury prevention and rehabilitation strategies.

As a Occupational Therapist

Qualifications:

• Licensed Occupational Therapist in Connecticut

• Reliable transportation, and a willingness to cover Norwalk and surrounding areas

• Proven experience in Occupational therapy, preferably in a home health or clinical setting

• Strong understanding of musculoskeletal and neurological conditions

• Excellent communication and interpersonal skills

• Ability to develop and implement individualized treatment plans

• Knowledge of adaptive equipment and assistive devices
