Job summary

Rehab Alliance in California is seeking a licensed Occupational Therapist for a per diem position in SNF and long-term care settings. You will evaluate and treat patients with functional and cognitive challenges and develop personalized care plans.

Qualifications

• Must hold a current Occupational Therapist license in the state of practice.
• NBCOT certification as an OTR where required by state.
• Licensed Occupational Therapist to apply.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all required physician orders are obtained and up to date.
