Optum Home & Community Care, part of Optum, is hiring an Advanced Practice Clinician to support high‑risk members in Senior Community Care settings. The role focuses on assessments, rehab support, documentation, and collaborative care planning to improve care transitions.

Active NP/PA board certification, Texas licensure, and DEA authorization are required, with LTC experience preferred. A supportive team and meaningful impact on patient outcomes are offered in a busy, patient‑centered

  • Active ANCC/AANP certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care.
  • Active, unrestricted Texas license or ability to obtain by start date.
  • 1+ year of clinical APC experience in long-term care or similar.
  • Active DEA licensure or ability to obtain post-hire.

Responsibilities

  • Conduct comprehensive assessments, including past medical history, meds, vital signs and exams.
  • Support members during short-term sub-acute rehabilitation episodes.
  • Ensure accurate documentation, including ICD-10 conditions.
  • Collaborate with interdisciplinary care team.
  • Conduct advanced illness and advanced care planning conversations.
  • Provide counseling and education to patients and caregivers.

The Senior Community Care (SCC) program is an integrated care delivery program that coordinates the delivery and provision of clinical care of members. The Advanced Practice Clinician within SCC provides care to our highest-risk health plan members. Primary Responsibilities:
  • Conduct comprehensive assessments, including:
    • Past medical history review, medication reconciliation, vital signs, and physical exam
    • Evidence-based screenings and point-of-care testing (as appropriate)
  • Support members during a short term sub-acute rehabilitation episode
  • Ensure accurate and complete documentation, including ICD 10 conditions
  • Communicate and collaborate with the interdisciplinary care team
  • Conduct advanced illness and advanced care planning conversations
  • Provide patients and caregivers with counseling and education

Required Qualifications:
  • Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
  • Active, unrestricted licensure in Texas (or ability to obtain by start date) without current disciplinary actions or practice limitations
  • 1+ years of clinical experience as an APC (long-term care setting preferred)
  • Active and unrestricted driver’s license
    • In addition to a driver’s license, access to reliable transportation
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
  • Ability to gain a collaborative practice agreement, if applicable in your state
  • Ability to navigate varied environments and to position oneself as needed to perform job duties
Preferred Qualifications:
  • Experience in geriatric medicine, long-term care, senior living or home care setting
  • Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end of life discussions
  • Proficiency with electronic medical records and technology

Compensation for this specialty generally ranges from $53 - $79 per hour. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
