Job summary

Aveanna Healthcare is hiring a Licensed Practical Nurse (LPN/LVN) for per diem home care. You will provide skilled nursing care under RN supervision in patients' homes, following care plans and physicians’ orders.

The role requires an active LPN/LVN license, CPR certification, and reliable transportation. We offer a caregiver-friendly environment with family-centered care, opportunities for skill advancement, and flexible shifts.

Qualifications

• Must be a graduate of an accredited school of nursing.
• Current, unrestricted LPN/LVN license in the state of practice.
• Current CPR certification.
• TB skin test current within last 12 months.
• 6 months hands-on nursing experience preferred.
• Reliable transportation.

Responsibilities

• Deliver and coordinate quality patient care per physician orders.
• Observe patient condition and report changes to supervisor/physician.
• Document nursing actions and care coordination promptly.
• Participate and update the nursing care plan.
• Take appropriate nursing actions based on assessment to achieve outcomes.
• Recognize changes requiring intervention and adjust care.
• Comply with Aveanna policies and regulatory requirements.
• Provide care with infection control per OSHA standards.
• Educate patient and family on disease, self-care and prevention.
• Maintain knowledge through ongoing education and skills evaluations.
