United Methodist Communities’ HomeWorks Staffing in Northern New Jersey is seeking a Staffing Licensed Practical Nurse (LPN) for per diem shifts. The role offers $35 per hour with day, evening, and night shifts, including weekends.

You will provide resident care per care plans, document observations, and collaborate with nurses and caregivers to support memory care and assisted living communities. This flexible role supports a variety of schedules while ensuring high-quality care.

Qualifications

• Active New Jersey LPN license.
• At least 1 year of experience in memory care, assisted living, or another healthcare setting.
• Flu vaccine for shifts inside a UMC community (exemptions may be requested).

Responsabilités

• Fulfill scheduled shifts as assigned and provide coverage in accordance with community needs.
• Deliver individualized care and support based on each resident’s care plan, preferences, and daily needs.
• Complete routine resident rounds, monitor well‑being, and respond promptly to requests for assistance.
• Maintain accurate, timely documentation of care provided, resident observations, and significant changes in condition.
• Communicate effectively with incoming staff to support continuity of care between shifts.
• Work collaboratively with nurses, caregivers, and other team members to support resident care and daily operations.
• Build professional relationships with residents while maintaining a safe and supportive environment.
