Job summary

Alvita Care is seeking compassionate, reliable LPNs for per diem floor staffing opportunities in Patchogue, NY. Most shifts are 3 PM–11 PM on weekdays and weekends, with flexible per diem hours based on facility needs.

Pay rate is $35–$40/hour, depending on experience and assignment. Responsibilities include administering medications, monitoring residents, and maintaining documentation per facility policies.

Qualifications

• Valid New York State LPN license required.
• Previous nursing experience preferred.
• Long‑term care or facility experience preferred.

Responsibilities

• Provide quality nursing care and support to residents.
• Administer medications and treatments within the scope of the LPN license.
• Monitor residents' conditions and report changes to the appropriate healthcare professionals.
• Maintain accurate and timely documentation using the facility's charting system.
• Follow facility policies, procedures, and infection control protocols.
• Collaborate with other healthcare professionals to promote resident safety and well‑being.
• Assist with other nursing duties as assigned within the scope of practice.
