Job summary

Qualifications

• Provides input to the RN for the plan of care by communicating relevant patient information.
• Provides direct patient care within scope of practice to implement the plan of care.
• Informs and instructs patients and significant others on health status and plan of care.
• Assists patient and family in discharge planning and home health maintenance.
• Documents all aspects of patient care.
