Job summary

The Mental Health Association of Rochester is seeking a Licensed Practical Nurse (LPN) to deliver patient-centered nursing care in a facility setting.

You will administer medications and treatments, collect and document patient information, monitor health status, and coordinate care with the healthcare team to support residents.

An active LPN license, current BLS/CPR certification, and the ability to meet physical requirements are required; PPD within the last year is expected.

Qualifications

• Minimum 1 year of facility-based experience.
• Ability to meet physical requirements, with or without accommodations.
• Current BLS/CPR certification.
• PPD (Tuberculosis test) within the last year.

Responsibilities

• Deliver patient-centered nursing care within the LPN scope.
• Administer medications and treatments.
• Collect and document patient information.
• Offer wound care and patient education.
• Monitor and report changes in patient condition.
• Assist with activities of daily living as needed.
