Job summary

Affinity Hospice is seeking a per diem Registered Nurse to support home health patients in California. You will review OASIS assessments, ensure accurate ICD-10 coding, and collaborate with clinicians to maintain compliant patient care.

Ideal candidates hold an active California RN license with 1–2 years of home health experience, strong communication skills, and reliable transportation. Per visit rate $140–$150; flexible scheduling and ongoing education offered.

Qualifications

• Active Californian RN license in good standing.
• 1–2 years of clinical home health experience.
• Working knowledge of OASIS assessments and ICD-10 coding.
• Knowledge of federal regulations and state licensure requirements.
• Strong communication skills and a keen eye for detail.
• Ability to work independently and manage priorities with minimal supervision.
• Proficiency with electronic health records.

Responsibilities

• Review OASIS assessments for completeness and compliance.
• Ensure accurate ICD-10 coding and sequencing for each patient.
• Collaborate with clinical staff to resolve documentation and data integrity issues.
• Monitor visit utilization and flag potential financial risks.
• Identify trends impacting agency outcomes and process measures.
• Conduct OASIS inter-rater reliability visits with clinical team.
