Job summary

Longwood Hospice is seeking a compassionate Per Diem Hospice Chaplain to provide spiritual and emotional support to patients and families in our North Shore SNFs. The chaplain travels to facilities to visit patients and works with the hospice interdisciplinary team.

The role emphasizes meeting diverse beliefs with respect, offering prayer and spiritual counseling, and supporting families through bereavement. Documentation and HIPAA compliance are essential.

Qualifications

• Education or training in theology, ministry, pastoral care, chaplaincy, or a related field.
• Previous experience providing spiritual or pastoral care preferred.
• Ability to travel to North Shore SNFs and work independently within a healthcare team.
• Commitment to compassionate, nonjudgmental support across diverse beliefs.

Responsibilities

• Visit residents in North Shore SNFs to provide spiritual and emotional support.
• Assess spiritual needs and develop individualized care plans.
• Collaborate with nurses, social workers, physicians, and staff in the IDT.
• Provide prayers, rituals, or counseling per patient wishes.
• Document visits timely and maintain patient privacy.
