Responsibilities

• Provide spiritual assessment and direct spiritual care to patients and families.
• Develop and coordinate the plan of care with the interdisciplinary team.
• Attend care conferences and participate in group meetings.
• Offer education and support to the care team on spiritual care.
• Recruit and engage community chaplains and volunteers as needed.
• Document services and maintain quality assurance records.
• Plan memorial services and provide funeral support when requested.
• Supervise spiritual care volunteers where assigned.
