Job summary

Pathways Healthcare, a hospice and home health provider in New Hampshire, seeks a per diem chaplain to deliver spiritual care across the Seacoast service area, including Exeter and surrounding towns.

You will assess spiritual needs, develop care plans, and provide direct support to patients and families while collaborating with a diverse interdisciplinary team and community clergy. Prior hospice experience and interfaith sensitivity are valued.

Qualifications

  • Graduate of an accredited seminary or theology school with BD/MDiv or equivalent.
  • Experience with death, dying, and supporting families.
  • Ability to work in an interdisciplinary team.
  • Comfort in an interfaith setting and respect for diverse beliefs.
  • Willingness to network with community clergy.
  • Understanding of hospice philosophy of care.
  • If home visits are required, a licensed driver with insured vehicle.
  • Hospice experience preferred.
  • Ability to provide funeral or memorial services.
  • Participation in quality improvement teams.

Responsibilities

  • Assesses spiritual needs and ensures timely services.
  • Develops the care plan through comprehensive assessment of identified spiritual needs.
  • Provides direct spiritual care to patients and families.
  • Liaises with community chaplains and counselors.
  • Maintains records for QA, program development, and policies review.
  • Documents services and ongoing communication with chaplains.
  • Attends patient care conferences as part of the interdisciplinary group.
  • Provides consultation and support to the interdisciplinary team.

Position Details:
  • Main Office location: Londonserry, NH
  • Service Area: Seacoast Team: Exeter, Durham, Raymond, Sandown, Danville, Kingston, Portsmouth, Hampton, Epping, Epsom, New Market, Newton.

Responsibilities:
  • Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner.
  • Facilitates the development of the individualized plan of care by participating in comprehensive assessment to meet identified spiritual needs.
  • Provides direct spiritual care to patients and families/caregivers.
  • Serves as liaison and support to community chaplains and spiritual counselors.
  • Maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision.
  • Documents direct services and ongoing communication with community chaplains and spiritual counselors.
  • Attends patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group.

Position Qualifications:
  • Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree). Experience working with death and dying, individuals/family/caregiver.
  • Ability to work as member of interdisciplinary group.
  • Comfort in an interfaith setting.
  • Ability to accept different lifestyles, cultures, beliefs, and values.
  • Ability to network with community clergy and congregations.
  • Knowledge of and commitment to hospice philosophy of care.
  • If making home visits, must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Hospice/experience preferred.
  • Provides funeral or memorial services for patients as requested.
  • Plans periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.
  • Assists in supervision of spiritual care volunteers when assigned to patients/ families/caregivers.
  • Actively participates in quality assessment performance improvement teams and activities.
