Job summary

Liberty Health is seeking a qualified HOME CARE SPEECH LANGUAGE PATHOLOGIST (SLP) to join our per diem team serving Halifax, Ahoskie and surrounding areas in North Carolina.

The role provides patient-focused evaluation and treatment of speech, language, swallow, and cognitive disorders under physician orders, with accurate documentation and coordination with branch staff. Travel is required as assigned, and prior supervision or mentoring may be provided.

Qualifications

• Master’s degree in speech language pathology and CCC-SLP with state licensure.
• Strong communication and organizational skills; CPR certification.
• Annual minimum of twelve hours of continuing education.
• Willingness to travel as assigned.
• Minimum 1 year therapist experience.

Responsibilities

• Provide patient care services with direct treatment and follow-up.
• Evaluate and treat patients with physician’s orders for speech, language, swallow, or cognitive disorders.
• Document care data accurately and timely, showing progress.
• Communicate with intercompany and branch staff to maintain open lines.
• Attend care conferences and provide weekly statistics.
• Provide in-service education and supervise peers and students as needed.
• Conduct research and assist with orientation of new employees.
