Job summary

UnitedHealth Group, through Assured Home Health/LHC Group, is hiring a Social Worker - Per Diem to provide medical social services under physician direction and the Interdisciplinary Group, helping patients and families understand social and emotional factors affecting health.

Requirements include a Master’s Degree in Social Work from CSWE-accredited program, a valid driver’s license with reliable transportation, and Oregon licensure as a Social Worker; CPR preferred.

Qualifications

• Master’s Degree in Social Work from a CSWE-accredited program.
• Current driver’s license, vehicle insurance, and access to reliable transportation.
• OR: Licensed as a Social Worker in Oregon.
• Preferred CPR certification or ability to obtain within 90 days.
• Licensed Social Worker in the state of residence.
• 1+ years of social work experience in a health care setting.

Responsibilities

• Provide medical social services under physician direction and Interdisciplinary Group.
• Assist in understanding social and emotional factors related to the patient’s health and development of coping mechanisms.
